HOW THE HEART'S ELECTRICAL SYSTEM WORKS

The heart is a muscular pump controlled by its own internal electrical system. In a healthy heart, electrical signals fire in an organized sequence, causing the chambers to contract (squeeze) in coordinated rhythm. When that electrical system is disrupted, the result is an arrhythmia (an abnormal heart rhythm). Arrhythmias can run in two directions: the heart can beat too slowly, or it can beat too quickly and chaotically. Each direction carries its own risks, and each calls for a different type of implanted device.

WHAT IS A PACEMAKER

A pacemaker is a small device implanted beneath the skin near the chest. Thin, flexible wires called leads connect the device directly to the heart muscle. The pacemaker continuously monitors the heart's rhythm and, when it detects that the heart is beating too slowly, it delivers a small, precisely timed electrical signal to prompt a contraction.


Pacemakers are most commonly used for bradycardia (a resting heart rate that is persistently too slow), heart block (a condition in which the electrical signal between the upper and lower chambers of the heart is delayed or interrupted), and certain conduction disorders (conditions in which the electrical pathway through the heart muscle does not function normally). Because the device only stimulates the heart when the natural rate falls below a programmed threshold, most patients are unaware of its activity during daily life.


WHAT IS A DEFIBRILLATOR (ICD)

An implantable cardioverter-defibrillator, commonly abbreviated as ICD, is designed for a different and more immediately dangerous category of arrhythmia. Where a pacemaker addresses rhythms that are too slow, an ICD monitors for rhythms that are dangerously fast or disorganized. Ventricular tachycardia (an abnormally rapid rhythm originating in the lower chambers of the heart) and ventricular fibrillation (a chaotic, quivering rhythm in the lower chambers that stops effective pumping entirely) can both cause sudden cardiac arrest if left untreated.


When an ICD detects either of these rhythms, it can deliver a controlled electrical shock to depolarize (electrically reset) the heart muscle and restore a normal, organized rhythm. ICDs are typically recommended for patients who have survived a cardiac arrest, who have severe weakness of the heart muscle, or who are identified as being at elevated risk for a life-threatening arrhythmia. HOW THESE DEVICES ARE SIMILAR

Despite their different purposes, pacemakers and ICDs share a common architecture. Both are implanted under the skin near the chest, typically just below the collarbone. Both use leads that are threaded through a vein into the heart. Both monitor heart rhythm around the clock. And both are designed to reduce the risk of serious rhythm complications over the long term. It is also worth noting that some modern devices combine pacemaker and defibrillator functions into a single implant, providing protection against both slow and dangerous fast rhythms simultaneously.
